Guillain-Barré
Syndrome*
Treatment.

Guillain-Barré Syndrome (GBS) is an acute autoimmune inflammatory polyneuropathy in which the immune system mistakenly attacks the peripheral nervous system, causing rapidly ascending weakness that can progress to complete paralysis and respiratory failure. The condition is typically triggered by a preceding infection (Campylobacter jejuni, cytomegalovirus, or influenza). While GBS is usually self-limiting, recovery can take weeks to years, and up to 20% of patients have persistent disability. At Bethesda Physio & Rehab Clinic, our GBS rehabilitation program covers the full recovery arc from early ICU mobilization, preventing contractures and ICU-acquired weakness, through progressive ambulation retraining, and ultimately restoring full functional independence. Our therapists work closely with the neurology team throughout the critical and post-acute phases.

Symptoms
  • Rapidly progressing weakness starting in the legs and ascending toward the trunk and arms
  • Loss of deep tendon reflexes (areflexia) which is a hallmark clinical sign
  • Severe neuropathic pain, particularly in the back and legs
  • Autonomic dysfunction including heart rate and blood pressure instability
  • Respiratory muscle weakness in severe cases requiring mechanical ventilation
Causes
  • Autoimmune molecular mimicry following Campylobacter jejuni gastroenteritis infection
  • Post-viral inflammation following cytomegalovirus, Epstein-Barr, or influenza infection
  • Rare post-vaccination trigger causing aberrant immune activation
  • Zika virus infection identified as a GBS trigger in endemic regions
  • Unknown idiopathic immune trigger in a subset of patients
Diagnosis
  • Clinical assessment using Brighton Collaboration Criteria for GBS diagnosis
  • Cerebrospinal fluid (CSF) analysis showing albuminocytological dissociation (high protein, normal cell count)
  • Nerve conduction studies (NCS) and EMG to identify demyelinating or axonal variants
  • Respiratory function monitoring (FVC, NIF) to assess ventilatory muscle weakness
* Evidence-Based Interventions

Clinical
Approaches

Early Passive Mobilization
Preventing contractures, deep vein thrombosis, and pressure sores during the acute paralytic phase.
Respiratory Physiotherapy
Breathing exercises, assisted coughing, and ventilator weaning protocols to restore respiratory independence.
Progressive Strengthening
Graded resistance exercises as nerve function returns, starting distally and progressing proximally.
Gait Retraining
Body-weight supported treadmill training, parallel bars ambulation, and orthotic support to restore safe walking.

Home Exercises Guide

Strengthen your recovery. Perform these exercises strictly under pain-free limits:

Execution Blueprint: Ankle Pumps & Passive Limb Movement (Acute Phase)

  1. 1Begin in the bed with the therapist supporting the limb throughout the exercise.
  2. 2Therapist slowly moves the ankle in full plantar and dorsiflexion rhythmically.
  3. 3Each joint of the lower limb is cycled through its full pain-free range of motion.
  4. 4Patient is encouraged to attempt active contraction even if no visible movement occurs.
  5. 5Repeat 10-15 cycles per joint bilaterally, 2-3 times daily to prevent contractures.

Common Questions

About 80% of GBS patients can walk independently within 6 months, and most achieve full or near-full recovery. However, 20% have persistent disability. Intensive, early physiotherapy greatly improves the completeness of recovery.
GBS can be life-threatening if respiratory muscles are severely affected, requiring mechanical ventilation in an ICU. Close monitoring of respiratory function (FVC) is essential during the acute progressive phase.
In the acute phase, physiotherapy focuses on passive joint mobilization to prevent contractures, positioning to avoid pressure sores, respiratory physiotherapy, and sensory stimulation to maintain nerve awareness.
Active strengthening begins only after the neurological deterioration has plateaued and stabilized, which typically occurs within 2-4 weeks. Premature vigorous exercise during the progressive phase can exacerbate weakness.
Yes, post-GBS fatigue is common and can persist for years even after motor recovery is complete. Energy conservation strategies, aerobic conditioning, and psychological support are important components of long-term management.
This depends on the degree of weakness at any given recovery stage. We progress from tilt table to parallel bars, rollator, quad stick, and finally independent walking as strength returns, adjusting AFOs and supports as needed.
Yes, neuropathic pain is present in up to 89% of GBS patients. We manage it through positioning, TENS, heat therapy, and gentle passive movement. Medications like gabapentin or pregabalin may be prescribed by the neurologist.
Classic GBS is monophasic (occurs once) in most patients. However, 1-3% may experience a recurrence. Chronic Inflammatory Demyelinating Polyneuropathy (CIDP) is a related condition with a relapsing course that requires different ongoing management.
