Sciatica
Therapy*
Treatment.

Sciatica refers to pain that radiates along the path of the sciatic nerve, which branches from your lower back through your hips and buttocks and down each leg. It typically affects only one side of your body. Our targeted neurodynamic mobilization and decompression strategies relieve sciatic nerve entrapment, eliminating radiating pain and tingling.

Symptoms
  • Radiating pain from the lower back through the buttock and down the back of the leg
  • Sharp, electric-shock-like pain that makes standing or walking difficult
  • Numbness, tingling, or muscle weakness in the affected leg or foot
  • Pain that worsens with coughing, sneezing, or prolonged sitting
  • A constant burning sensation typically localized on one side of the lower body
Causes
  • Herniated or slipped disc in the lumbar spine compressing nerve roots
  • Bone spurs (osteophytes) on the spinal vertebrae encroaching on the nerve path
  • Piriformis syndrome, where the piriformis muscle in the buttock spasms and compresses the nerve
  • Lumbar spinal stenosis, which is the narrowing of the spinal canal in the lower back
  • Spondylolisthesis, causing one vertebra to slip forward over another
Diagnosis
  • Straight Leg Raise (SLR) test to verify sciatic nerve root tension
  • Slump Test to evaluate neurological sensitivity
  • Motor testing of the foot (heel walking and toe walking) to assess nerve conduction
  • Reflex testing (Patellar and Achilles) to detect neurological deficits
* Evidence-Based Interventions

Clinical
Approaches

Nerve Mobilization
'Nerve flossing' exercises to slide the nerve through tight tissues, reducing irritation.
Lumbar Decompression
Manual or mechanical traction to open up compressed nerve root exits.
Piriformis Release
Dry needling and deep tissue release of the piriformis muscle to stop local compression.
McKenzie Extension Protocol
Targeted exercises to centralize radiating pain back into the lower back.

Home Exercises Guide

Strengthen your recovery. Perform these exercises strictly under pain-free limits:

Execution Blueprint: Sciatic Nerve Flossing

  1. 1Sit upright in a chair with both feet flat on the floor.
  2. 2Slump your shoulders forward and look down toward your chest.
  3. 3Slowly straighten your affected leg while simultaneously tilting your head back to look at the ceiling.
  4. 4Bend your knee to return foot to floor while lowering your chin back to your chest.
  5. 5Perform this oscillating flossing movement 10 times in a slow, controlled flow. Do not hold the stretch.

Common Questions

This is called 'pain centralization' and is a highly positive sign of recovery! It indicates that the nerve root compression in your spine is reducing, drawing the pain back toward the source before it completely resolves.
For the first 48 hours of an acute flare-up, use ice packs on your lower back for 15-20 minutes to reduce local nerve root inflammation. Afterward, alternate with moist heat to relax severe muscle spasms.
Rarely, but it is possible if nerve compression is severe and prolonged. Seek immediate emergency care if you experience progressive leg weakness (foot drop) or a loss of bowel or bladder control.
Nerve flossing is a gentle neurodynamic exercise that glides the sciatic nerve back and forth through its surrounding soft tissue tunnels, reducing adhesions and soothing irritation.
Piriformis syndrome is a condition where the piriformis muscle in the deep buttock spasms and compresses the sciatic nerve that runs directly beneath or through it, simulating spinal sciatica.
Yes, sitting increases intradiscal lumbar pressure by 40% compared to standing, and direct compression on the gluteal region can further entrap the sciatic nerve.
Yes, gentle, short walks on flat surfaces stimulate blood flow, pump nutrition into spinal discs, and keep the lower body joints from tightening up.
Yes, inserting sterile needles directly into deep gluteal trigger points releases the piriformis spasm, immediately taking mechanical pressure off the sciatic nerve.
We utilize the Straight Leg Raise (SLR) test and the Slump test, which dynamically stretch the neural tissue, alongside motor and reflex assessments.
Most moderate sciatica cases resolve within 4 to 8 weeks of targeted decompression, neural mobilization, and core stabilization therapy.
